Distinguishing Between Crepitus and Injury

For some people, walking up the stairs sounds like a percussion section. But is a noisy ankle a bad ankle?
The medical term for joint noise is Crepitus. The good news is that noise without pain is usually harmless. But noise with pain (or a feeling of catching) requires attention. Fulshear Foot & Ankle explains in detail below.
Scenario A: The Harmless Pop
If you hear a pop when you rotate your ankle, but it feels fine, it is likely:
- Gas Bubbles: Nitrogen bubbles popping in the synovial fluid (like cracking your knuckles).
- Tendon Snapping: A tendon sliding over a bony prominence. As long as it glides smoothly, it’s just physics.
Scenario B: The Painful Click (When to Worry)
If the click comes with an ache, swelling, or a feeling of “instability,” it could be:
1. Peroneal Tendon Subluxation
The tendons on the outside of your ankle are supposed to stay behind the ankle bone. If the retainer (retinaculum) is torn—often from an old sprain—the tendon snaps over the bone with every step.
- The Sensation: A visible “snap” on the outside of the ankle.
- The Risk: This creates friction that tears the tendon over time.
2. OCD Lesion (Loose Body)
A chip of bone or cartilage might be floating inside the joint space due to a past injury.
- The Sensation: The joint “locks up” or catches, then clicks when it releases. It acts like a pebble in a gear.
- The Risk: This loose body grinds away healthy cartilage, leading to early arthritis.
3. Scar Tissue
- Old injuries create scar tissue (fibrosis) that is thicker and less flexible than healthy tissue. It snaps when stretched.
